Our appointments are scheduled exclusively for each patient. This means that Dr. Baksh and/or the hygienist will be reserved for you.
As a service to our patients, our scheduling coordinator will email or call to reminder you the day before an appointment, upon your request. For continuing care appointments, our scheduling coordinator will send an email or call with a reminder, 2 weeks in advance, since we schedule these appointments 3-9 months ahead. We would appreciate a response to these emails or calls in order to confirm that you will be attending the appointment.
In the event an appointment must be rescheduled, please notify our office at least 48 hours in advance to avoid a missed appointment charge. The reserved appointment time can then be offered to another patient. If we are able to fill the appointment the missed appointment fee will be waived.
For your convenience, you may request an appointment and complete the necessary forms.

Financial Matters
For your convenience, we accept Cash, Debit, Visa, and MasterCard. Payment is due when services
are rendered. For those services that require a greater investment in your oral health, we
have alternate payment options. Please feel free to discuss them with us.
Dental Benefits
For the convenience of our patients, we can submit dental claims electronically or we can provide
you with a form after each visit. Dental benefits are a contract between you and your insurance
company, therefore your insurance company will reimburse you directly. Our treatment recommendations
are based on your needs not on your benefit plan.
Please remember you are responsible for all fees charged to you regardless of your dental
benefit coverage.
